Note: There is a microwave and a refrigerator on site, which your child is more than welcome to use. If you do plan to pack lunch for your child, please label each item with your child’s name. Acting Out will not be providing lunch for any students; however, there are two delis on our block where your child may purchase food. In order for your child to be allowed to leave our premises, we need your permission in writing. Please check one of the following (required):

DEPOSITS/PAYMENTS: A $500 deposit or payment in full must accompany the registration for Musical Theater Boot Camp. This cost is fully refundable with no additional cancellation fees until April 30th. After April 30th, there will be a fee of $50 assessed for cancellation. You must pay the balance in full by the first day of your chosen session, or by the first day of the first session (July 5th) if you're choosing both sessions. If you register after the first day, payment must be made in full at the time of registration. After classes begin, there will be no reduction of fees due to absence, late arrival, late registration, withdrawal, or dismissal.

Please read each of the following carefully:

1. I, the parent/guardian of the above named student, hereby give permission to Acting Out, its agents, representatives, and employees, to enroll my child in all activities offered by Acting Out’s Musical Theater Boot Camp except those activities in which I have recommended that the student not participate. In consideration of my child’s participation in Acting Out’s Musical Theater Boot Camp, I the undersigned waive all claims for damages I may have against Acting Out, its directors, faculty, and employees for any and all injuries suffered by my child. I also give permission to Acting Out, if its staff is unable to contact me, to take any necessary steps to obtain proper treatment of my child in the event of a sudden illness or injury. I understand that every effort will be made to notify me immediately in case of such an emergency. I further agree to be totally and completely responsible for the payment of all debts, expenses, or bills incurred in connection with any illness or injury of my child.

2. Consent is hereby expressly granted for the collection, retention, and use of written, auditory, photographic, videographic, and other materials pertaining to our/my child’s activities during the program at Acting Out’s Musical Theater Boot Camp, for commercial and educational purposes in print and on the Internet, provided that Acting Out shall retain control of, and have complete responsibility for, any use thereof.
